Client Consent Form

Please note: If you are under the age of 18 you must bring a parent or legal guardian to sign this form. We will NOT accept signatures from anyone under the age of 18. Please let us know ahead of time if your parent/legal guardian is not available for your appointment and we can email/text it to them.

I understand and agree to the following:

I am over the age of 18.

I understand that chemicals and the products used during a service can have varying effects based on each individual's physical characteristics. I am electing to receive these services and that an allergic or adverse reaction is possible.

I agree that Weave Central LLC and its employees are not responsible or liable if I have an allergic reaction or adverse reaction to any of the products or techniques used.

I agree that I will follow the regimen and the suggested follow-ups received from Weave Central LLC and it's employees with regard to the health, treatment, and maintaining my hair.

I agree that I will not go over the advised wear time frame for my service(s) and if I do, Weave Central LLC and it's employees are not responsible for any damages or injuries that may occur.

I acknowledge that I have been fully informed by my hairstylist and understand and assume all risks associated with the service I am receiving.

I confirm that Weave Central LLC and it's employees will not be responsible or liable if the result of the service is not as expected.

I understand the price of the service(s) and understand that there are no refunds.

I agree and release Weave Central LLC and it's employees from any and all Liability and responsibility for any damages or injuries to myself or my personal property.

I give consent to Weave Central LLC to take photographs and videos of the provided service.

I consent that Weave Central LLC may use my photos in terms of sharing the photographs and videos on social media for marketing campaigns or testimonials.

Weave Central LLC serves clients one (1) alcoholic beverage free of charge. Weave Central has no responsibility or liability of any injuries or damages caused by clients consuming alcohol. I hereby waive all legal rights to pursue any form of legal action against Weave Central LLC resulting from any injuries or damages caused by consuming intoxicants.

I have read and understood the terms and conditions of this waiver and release. I understand and confirm that by signing this waiver and release, I have given up considerable future legal rights. I have signed this agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me. My signature is proof of my intention to execute a complete and unconditional wavier and release of all liability to the full extent of the law. I am 18 years of age or older and mentally competent to enter into this waiver and release.

All deposits/payments are non refundable. If you make an error as a result of not reading or typing the wrong info, your payment is not refundable. All of our services describe exactly what each service entails. So please be sure to read the description. Please check this box if you agree.

Do you consent to our refund policy stating that we do not offer refunds and all sales are final?

Please note anything extra you need. For example if you need a take down and shampoo, please note it below. If you state that you need something then show up not needing it, you will still be charged for that service. Let's not play with each others time.

There are no children or extra guest allowed. We have limited space so any extra guests will be asked to wait in the car or lobby. We do not allow children in the salon at all. Your appointment will be canceled and you will not be refunded if you show up with children.

Please be sure to come with no product on your hair. THIS INCLUDES LEAVE IN CONDITIONERS, OILS, MOISTURIZERS!!! We will gladly oil your scalp once we finish braiding. Products will prevent lace from adhering or make your leave out oily and flat. We will not be held responsible if you fail to follow these instructions.

Are you allergic to anything? This includes braiding hair, yarn, lace glue, gel, hair spray, edge control? Please note your allergies below. If you are allergic to braiding hair or yarn please bring your own braiding hair.

If you book the wrong service by not reading or calling us, you still will be charged the full price of whatever service you booked. For example, If you mistakenly book a lace closure sew in which is a 3 hour service and come in saying that you want a basic quick weave which take an hour and 1/2 you will still be charged the full price of the lace closure sew in. Because when you book that blocks off our time and we're not able to accept more appointments for that time. Please book the right service.

If your hair needs to be detangled let us know. There is an additional detangling fee. WE WILL NOT TAKE DOWN A QUICKWEAVE THAT WAS NOT DONE BY US.

DO NOT SHOW UP WITH DIRTY HAIR OR A LOT OF PRODUCT IN YOUR HAIR EXPECTING US TO WORK ON IT. IT IS AGAINST STATE LAW TO DO SO. If you don't need a shampoo there should be not product in your hair.

Please DO NOT arrive early unless told to do so. Stylists are on an on-call basis and only show up once an appointment has been booked. So if you arrive an hour or half an hour early there is a chance no one will be there. WE WILL START AT THE TIME OF YOUR APPOINTMENT! IF YOU ARRIVE EARLY DO NOT EXPECT FOR US TO START ON YOU RIGHT AWAY. We also have to have time to set up and clean in between clients. Just arrive on time. There is only a 10 Minute grace period to get to your appointment.
